In response to BAWs request I am providing a list of great venues in Perth, Western Australi.
MOJO's Bar - North Fremantle - Live Orignial
The Grosvenor - Central Perth City, Live Original venue used to promote original music and for the release of CD's and EP's
The Amplifier Bar - Perth City, Great Live Venue for upcoming bands. Undoubtedly the best venue to launch a new CD release in Perth.
Taipan Room - North Bridge, Perth City - Night Club atmosphere where orignal bands can host their gigs and debuts
The Craic - Perth Metro Area - great night out where some of the best Cover Bands strut their stuff
Burswood Dome - Perth City - Hosts world class and national acts. for example Darren Hayes of Savage Garden will play to 5000 fans in two weeks time
Peninsula Hotel - Mandurah 65km south of Perth. Large venue holds approx. 2000. Host for medium level national acts such as Regurgitator, The Cruel Sea, Crowded House etc.

Whitesides, I'm not sure what you mean't I think you were trying to say Venue Form??? Are you after Venue Addresses and phone number details??
