It has come to my attention that at least one of you out there had submitted your questionnaire and for some reason I did not receive it. So, I am asking for you to fill out the below questionnaire and send it in to me. If you want me to check and see if I have yours already then send me an email and I will check. This will save you the time of filling another one out. But I do highly recommend you fill this out to be considered for a Hallenbeck's Showcase. This is the only way I know who is interested in performing.

Also, please pay attention to the specific genre that Hallenbeck's Showcase is geared toward as listed below. You may submit your questionnaire if you do not fit into this genre anyway for later consideration in your particular genre once we have a venue and established the Showcase.

Please respond to this email if you are interested in performing in the showcase at Hallenbeck's General Store in No. Hollywood. The May Showcase has already been selected.

If you have already performed in a Showcase then disregard this email as you are not eligible for another Showcase for 3 months after your performance date.

If you are Hosting (Showcase or Open Mic), you are not affected by the same rule of 3 months. You may host one month and perform the next or perform one month and host the next.

You only have to fill out the questionnaire ONCE. The committee will have it on file for future Showcases selections.

Please fill out the Questionnaire fully as this will help the Committee decide the line-up for the Showcase. Also, when asked for your Bio, do NOT send us a link to your website as we will not have time to look at everyone's bio. Please take the time to give us a 50-word bio or less for a press-release.

Hallenbeck's is geared toward the singer/songwriter, folk, soft rock, pop, blues, country, lite jazz, etc. If you do not fit within these genres then help us out and find a venue that fits your music. Obtain the information for the Committee and we will follow up with the venue. The Showcase Committee will work hand in hand with you to work out all the details.

If so, what are they?
Have you performed in a JPF Showcase within the last 3 months?
If yes, which one?
Name the last JPF Activity you participated in. (ex: Meetings, Committees, Showcases (performer or host), etc.)
Would you be willing to HOST either the JPF Showcase or the Open Mic after the Showcase in the future?
Would you like to be a Showcase Coordinator in the future?
Anything you would like to add?

Please submit a 50-word bio that will be used in a Press Release to promote the JPF Showcase and performers. Also, do not send a link to your website so that I can obtain the bio material. Reason being, if every performer sent me links to their website I would be at the computer instead of at the Showcase. There is not enough time to visit every performers website and gather the bio material. You know what you would like the public to know about you more than I do.

It is highly, recommended that if once chosen to perform at the Showcase and something happens you cannot honor your commitment, then you are required to call me or the Showcase Coordinator as soon as possible. If you do not notify the Showcase Coordinator, or me as soon as possible, your future Showcase opportunities can be affected.

The Showcase Committee and I understand that these are short, free performances, and no artist who cancels with proper notice will have that cancellation held against them. An artist who simply does not show up for a showcase, without proper notice or explanation, will still be considered for any future showcases, but will be "last in line" on the list of submissions.

I welcome any feedback on the Showcase process, the Questionnaire, submission of a bio, etc.
